Team Checklist

1. Complete the tournament roster online using our Team Management tools (above).

  • »  Preliminary roster is due June 1, 2009.
  • »  Addresses required; addresses are directly linked to the NSC Liability Waiver as required by the State of Minnesota.
  • »  Roster changes may continue until the following dates:
    • »  Wednesday, July 7 at 1600: Schwan’s USA CUP Weekend
    • »  Sunday, July 11 at 1600: Schwan’s USA CUP
    • »  Final rosters will be printed on these dates/times for Final Team Check-in and will not reflect player addresses.
    • »  If a player must be added after these dates/times, print an updated roster and bring it to Final Team Check-in.
    • »  Players dropped at Final Team Check-in will receive a refund of the player fee following the tournament.
  • »  A unique jersey number is required for each player; no jersey sharing permitted.
  • »  Provide each player’s primary position on final roster.
  • »  ALL guest players must be identified on the roster. 
  • »  Players 16 and above may participate in the College Coaches Recruiting Program.
    • »  Players may provide the team leader with their email address, grade point average (GPA), graduation year, position, and SAT or ACT scores for the roster.
    • »   At a minimum, the player’s email address and graduation year are requested by the coaches for participation.
    • »  Players 15 and younger will leave these fields blank

2. Direct parents to complete the online NSC Liability Waiver (above).

  • »  Provide parents with the tournament ID number for this task. Players are not permitted to participate in the tournament without a completed NSC Waiver on file.
  • »  You may monitor the progress of your parents by selecting View Roster from the online roster menu. The WV column stands for Waiver. Each time a parent completes the online waiver, an ‘X’ is displayed in this column to the left of the player’s name.
  • »  Waivers are tracked to each player’s address, not the parent’s email address. Therefore, a parent may complete the online waiver from any computer!

3. Complete the Team Player Information Form.

  • »  This form is provided by MYSA, see http://www.mnyouthsoccer.org to print a copy of this form.
  • »  This form is a typeable format and will be used by Minnesota teams participating in non-US Youth Soccer events, which includes all 2009 NSC events.
  • »  This form provides verification of your players’ birth dates and current registration/standing with MYSA, to include your Minnesota guest players.
  • »  Write your tournament ID number/event in the upper right-hand corner of the form BEFORE sending to MYSA for approval.
  • »  Indicate how you would like the form returned. You may elect to have MYSA return the form directly to the tournament: Schwan's USA CUP fax, 763-785-3660 or USA CUP email at sleahey@usacup.org.
  • »  The NSC provides secondary medical insurance for all participants at the same level as MYSA. See certificate at the top of this page.

4. Guest Players

  • »  All guest players must be currently registered/in good standing with MYSA or any other FIFA-affiliate.
  • »  All guest players must be identified on your tournament roster. Identification occurs when you add your player information to the online roster.
  • »  Canada: Direct your guest player to contact his/her province/club for permission-to-travel or a player loan form.
  • »  United States
    • »  Direct the player to contact his/her State Association for permission-to-travel or the USYS Team Player Information Form per the State Association procedures for players/teams attending non-US Youth Soccer events.
    • »  Minnesota players will provide the USYS Team Player Information Form.

  • »  The NSC provides secondary medical insurance for all participants. See certificate on our sanctioning page.
  • »  Guest Player Limit: Up to 6 guest players may be rostered per team.

5. Player passes are required for play

    • » Minnesota teams and coaches must complete the NSC Player Pass Form.
    • »  Signature must match name on card.
    • »  Player passes must be laminated.
    • »  Copies of player passes are not accepted.
    • »  Canadian booklets/provincial player cards and FIFA-affiliate player passes are accepted by the National Sports Center for non-MN guest players.

6.  Player fees are payable by Visa, MasterCard, check, money order or cash. Team payment only; no individual payments.

  • »  To pay by Visa or MasterCard, use the online balance due payment option (available in the Team Management tools at top of page).
  • »  If you are paying by check or money order:
    • »  Make payment payable to either Schwan’s USA CUP Weekend or Schwan’s USA CUP.
    • »  Record your tournament ID number on check or money order.
    • »  Mail payment to 1700 105th Ave NE, Blaine, MN 55449
  • »  If you are paying by cash, office hours are Monday-Friday, 0900-1700. The main office is located in the Administration Building on the NSC campus; enter the building from parking lot B off Davenport Street, go to plaza level.
  • »  Players dropped from the roster at Final Team Check-in will receive the player fee refund following the tournament.
  • »  No invoice will be sent to your team.
  • »  For player fee breakdown, visit our fees section. Player fees are determined by the number of players on your roster.

7.  Hotel lodging, if applicable.

  • »  Teams are required to stay in the official tournament hotels.
  • »  Provide the name, city and phone number of the hotel (available in the Team Management tools at top of page). Update as needed.
  • »  Hotel contact information is used in an emergency. In the event that we are not able to contact you using your cell phone, we will contact you at the hotel.
  • »  The National Sports Center is also required by the State of Minnesota to track for economic impact. Did you know that the National Sports Center is second only to the Mall of America for economic impact and the number of annual visitors?

 

8.   Print the rules. Coaches are responsible for knowing the rules in advance. Express concerns prior to the start of each game.
